Request a Book from a Chicago Campus Library

Use the form below to request books from Chicago campus locations ONLY (Schaffner Library, Galter Health Sciences Library, and the Law Library (Pritzker Legal Research Center), for pickup at Evanston campus locations (Main Library, Science and Engineering Library, or United Library).

1. Fill in all the sections under Personal Information.
2. Fill in all or as much of the Book Information as you can. Title, Call Number, and Author are required.
3. When you have completed the form click on Submit Request, or, if you wish to start over, click on "Clear the Form".

This form is not intended for requesting items that are checked out by another patron. To recall a checked out item, please use the Recall Form.

If you are on the Chicago campus and would like to place a request for a book from the Evanston campus, you may inquire at the Chicago campus branch nearest you.
