University of Chicago Borrowing Privileges Application

This form is for current Northwestern University students, faculty and staff who would like to apply for borrowing privileges at the University of Chicago libraries. Please read the NU/ U of C reciprocal borrowing policy [1] if you have not already done so.

Please Note: You can get a card instantly, by going directly to the Library Privileges office at U of C's Library. Using this form means you will have to wait 5-7 days before your card is available. The patron must pick up the card in the Northwestern Library Privileges Office.
