Use the form below to request materials that have not yet been fully cataloged.

  • Before you fill out this form, please check that the NUcat [1] record indicates the item's current status as "In Process," "On Order," or "Received on [date]; Inquire at Circulation Desk."
  • Materials that are in process or uncataloged from the Music Library [2], the United Library [3], the Galter Health Sciences Library [4], or the Law Library [5] CANNOT be requested using this form. Please contact the owning library directly for assistance.
  • If you are requesting a video, you must choose the Mitchell Multimedia Center as the pick-up location.
