  2. Fill in all or as much of the Article Information as you can.
  3. When you are done, click on Send your Article Request to submit the form to the circulation supervisor.
  4. If you wish to make another request, click the "Start Over" button. This will clear the form and you can start over or cancel at this point.

For journal articles available at Galter Health Sciences Library or Law Library (Pritzker Legal Research Center), please go directly to those libraries. Do not use this form.
