Request an Item from the Northwestern Libraries
to be delivered to your home or office

This service is currently available to only to students in the online Master of Science in Medical Informatics program.

Instructions

  1. Please fill out both sections of this form, providing as much information as you can about the item you are requesting.
  2. Northwestern materials currently available in the libraries will usually be shipped to you in about 3 business days. Items currently checked out to another user will usually be available to ship in about 2 weeks.
  3. You will receive an email when your material has shipped. This message will include a UPS tracking number.
  4. Your requested material will arrive with a prepaid return shipping label. You may reuse the box or envelope in which the material was shipped to you to return it.
  5. Your item(s) must be received at the Library by its due date, or overdue fines will accrue. Please plan to ship the item(s) back to the library in advance of the due date to account for transit time.

Additional instructions and detailed information about Northwestern University Library circulation policies will accompany your requested material.
