Use the form below to request materials that have not yet been fully cataloged.

• Before you fill out this form, please check that the NUcat record indicates the item's current status as "In Process," "On Order," or "Received on [date]; Inquire at Circulation Desk."
• Materials that are in process or uncataloged from the Music Library, the United Library, the Galter Health Sciences Library, or the Law Library CANNOT be requested using this form. Please contact the owning library directly for assistance.
• If you are requesting a video, you must choose the Mitchell Multimedia Center as the pick-up location.
